Surgery/Anesthesia Consent Form
Clients admitting their pets for surgery are required to sign a Surgery/Anesthesia Consent Form at the time of check-in. The Surgery/Anesthesia Consent Form outlines the condition requiring treatment and the nature, purpose, risk and benefits of the operation(s)/procedure(s). Please read this form carefully and allow time to discuss it with one of our team members at the time of admission.
Dental Consent
Clients admitting their pets for dental procedures are required to sign a Surgery/Anesthesia Consent Form and a Dental Procedure Detail Form at the time of check-in. The Dental Procedure Form outlines common dental problems and possible therapeutics that may be needed. Please read both forms carefully and allow time to discuss them with one of our team members at the time of admission.
